Keep in mind that transition goals can change several times as a student`s interests develop. It`s not uncommon for teens to explore different careers and paths. Transition objectives are reviewed and updated annually with the rest of the IAP. An appropriate formal and informal assessment of a student`s strengths, interests, preferences and needs should be conducted. The assessment should address the three main areas of transition planning and take into account the student`s age, disability and cultural background. Such considerations allow school staff to obtain accurate and valid information about the student. In addition to goals and services, almost all MYP transition plans include activities for the student. The idea is to prepare teenagers for adulthood. One thing you can do is think about what you want to do after high school. Get ideas: Parents` rights under IDEA are transferred to the disabled child at the age of majority (18 in most states), unless the child is deemed incompetent. The child and the parents must be notified by the school. If you are a student with an IEP, you can expect the school to invite you to be part of the transition planning. Students are encouraged to play a leading role in the transition to the MYP. In fact, the MYP team must invite a student to meetings where transition planning is discussed. If the student is unable to attend, the team must ensure that the student`s interests and wishes are taken into account. The Education of Persons with Disabilities Act (IDEA) is the federal Special Education Act that ensures that all children with disabilities have access to free and adequate public education. The IDEA recognizes the importance of preparing youth for success after high school and states that transition planning for students who receive special education services and have an Individualized Education Program (IEP) must begin at age 16 (some states require the process to begin earlier). Transition planning is about assessing the needs, strengths and skills a student needs to transition from high school to post-secondary life. Transition planning is required by law for all students who have an IEP. Transition planning should begin with the first IEP, which comes into effect when the student reaches the age of 16 or younger when the MYP team deems it necessary. For students to achieve their measurable goals after school, the IEP must include a description of the transition services required to help the student transition from high school to the adult world.
